A Burst Pipe Can Be Repaired, a Burst Artery Often Cannot

Last week, our clinic experienced an unexpected plumbing disaster. What began as an ordinary workday suddenly turned into hours of disruption. A hidden pipe burst without warning, flooding the bathroom and requiring thousands of dollars in emergency repairs. No one expected it.

Then, only days later, the nation was saddened by the sudden passing of Senator Lindsey Graham, who reportedly died from an aortic dissection caused by arteriosclerotic cardiovascular disease—a tear in the body’s largest artery resulting from years of silent damage.

Although these events seem unrelated, they share an important lesson: a building’s plumbing can often be repaired, a ruptured artery usually cannot.

The Plumbing Inside Your Body

Your cardiovascular system is remarkably similar to the plumbing in a building. Healthy arteries carry oxygen and nutrients to every organ, just as pipes deliver water throughout a home. When the pipes are clean and strong, everything flows smoothly.

But over many years, cholesterol particles, inflammation, elevated blood pressure, excess blood sugar, smoking, chronic stress, and poor lifestyle habits slowly damage the inner lining of our arteries. Like corrosion inside old plumbing, this damage often develops silently. You don’t see it, feel it until one day…a heart attack, stroke, aortic dissection, or sudden cardiac death. Unfortunately, our arteries usually don’t leak first to warn us.

Heart Disease: America’s Silent Killer

Cardiovascular disease remains the leading cause of death in the United States, claiming nearly 800,000 lives each year. Most people are familiar with cholesterol numbers: Total cholesterol, LDL, HDL, and triglycerides. These tests remain useful—but they don’t tell the whole story.

Imagine traffic on a freeway. LDL cholesterol (LDL-C) tells us how much cargo all the trucks are carrying. But Apolipoprotein B (ApoB), a key indicator, tells us how many trucks are actually on the highway. From the standpoint of damaging the road or your arteries, the number of trucks matters more than how much cargo each one carries.

Every ApoB-containing particle has the potential to enter the artery wall and contribute to plaque formation. That is why preventive cardiologists now consider ApoB to be a more accurate indicator of cardiovascular risk than LDL cholesterol alone.

Then There Is Lipoprotein(a). If ApoB counts the trucks, then Lipoprotein(a) or Lp(a) identifies a special kind of truck. Think of it as an armor-plated truck carrying highly dangerous cargo. Lp(a) is largely inherited. You are essentially born with your Lp(a) level, and lifestyle has only a limited effect on it. However, some studies have found that nutrients such as coenzyme Q10 and niacin can lower Lp(a). 

People with elevated Lp(a) have a substantially greater lifetime risk of coronary artery disease, stroke, calcific aortic valve disease, and other cardiovascular conditions, even when their traditional cholesterol numbers appear normal. That is why we believe everyone should have Lp(a) measured. 

Looking Inside the Pipes

Blood tests are only part of the picture. The most important question is: Has plaque already begun forming? Modern screening allows us to look directly inside the arteries before symptoms develop. Depending on your age and risk factors, this may include:

  • Coronary artery calcium (CAC) score

  • Carotid artery ultrasound

  • Echocardiogram (heart ultrasound)

  • Comprehensive cardiovascular laboratory testing, including ApoB and Lp(a)

These tools help identify disease years before it causes a heart attack or stroke.
